Registration Forms
Below are the three forms you will need to fill out to become a registered member of Wellesley Masters Swimming. These forms are required even if you are signing up as a daily member. Please bring these forms with you within the first week of practice you attend.
Wellesley College Liability Form – print, sign, and bring with you to practice
USMS Registration – please register on-line by visiting www.usms.org, clicking on the red “join” button, type in their zip code then click the “register on line” link. Make sure you are registering with WELL.
(Swimmers must have a USMS membership to swim in our program)
